CHHN provides in-home, skilled and unskilled services to seniors, older adults, and those who are disabled. Utilize the nursing process to assess, plan, implement, and evaluate patient care. On each assignment, provides skilled nursing care/services in accordance with prescribed orders Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications

Valid nursing license (required) 1+ years of licensed practical nursing experience Associate degree in nursing (required) Home health experience is a plus.
