THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES

1. Carry a high-quality caseload: Ensure visit quality, with OASIS accuracy that holds up to review.
2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.

REQUIREMENTS

• Active Michigan RN license
• Reliable transportation, valid driver's license, auto insurance
• Home health / OASIS experience strongly preferred THE REST OF THE PACKAGE - $1k bonus for referrals (we’re growing)!
