Key Responsibilities

Complete ongoing interdisciplinary assessments and develop individualized care plans Provides services that are ordered by the physician as indicated in the plan of care. Coordinate care with interdisciplinary team members Educate patients and caregivers on disease processes, medications, and safety Maintain accurate, timely documentation Communicate effectively with physicians, families, and internal team members Develops treatment program and establishes goals for improved function.
