• We are looking for a Clinical Team Support to provide administrative and logistical support to the Clinical Managers and their clinical teams and assist in coordinating the seamless delivery of patient care. The CTS is responsible for the day-to-day management of the Certified Nursing Assistant (CNA) schedule and provides front-line phone support for all calls coming into Hospice of the Chesapeake. Office Location: Largo, Maryland Status: Full-Time, Monday
• Friday 7am - 3:30pm. Salary: Competitive, commensurate with experience, education and credentialing Clinical Team Administrative Support Assists the Clinical Manager with compiling and/or producing reports in an accurate, efficient, effective, and timely manner, and maintains computerized and hard copy files for the Clinical Manager as requested. Faxes IDT care coordination notes to the attending provider and/or facility. Prints medication lists and IDG notes for the RN to pick up. Schedules and manages monthly Face-to-Face visits for providers. Manages DME pick-up. CNA Scheduling & Productivity Coordinates CNA schedules to provide safe, therapeutic, and efficient patient care to HOC clients based on location and appropriate skill set. Ensures weekly verification of visits in the EMR. Front Desk & Phone Support Provides front-line phone support for all calls coming into HOC: makes, receives, and relays calls and messages efficiently and assures appropriate follow-up. Prioritizes and responds to incoming calls as needed, triaging based on urgency and necessity. Frequently checks voicemail and email and keeps department staff apprised of urgent requests and situations. Manages front desk reception. Professional Practice Conveys positive and effective communication and customer service with internal and external clients. Demonstrates the ability to handle multiple priorities effectively and efficiently. Completes all job functions per department and organization policies and procedures. Attends mandatory in-services and/or meetings, or makes arrangements with the appropriate supervisor for satisfactory compliance. Performs other duties as required.

Qualifications

Skills & Qualifications Education & Experience High school diploma or equivalent required. Minimum of two years of experience in a customer service-related position; experience in a non-profit health care setting preferred. Full understanding of the role and responsibilities of a certified nursing assistant in the hospice environment.

Technical Skills Proficient in basic computer skills, including but not limited to Microsoft Word, Outlook, Excel, and scheduling software. Strong data entry skills with attention to detail, deadlines, and accuracy. Effective oral, written, and interpersonal communication skills. Ability to handle multiple assignments and projects simultaneously with attention to detail, efficiency, accuracy, and deadlines.

Requirements

Valid driver's license with a good driving record.
