On Call Pharmacist

Shifts would be after pharmacy closes, starting at 8pm.

**Key Responsibilities**

Provide on-call pharmaceutical support for long term care facilities as needed. Review and verify medication orders remotely to ensure accuracy and compliance. Consult with healthcare providers and staff regarding medication management and safety. Maintain closed door pharmacist responsibilities, ensuring patient confidentiality and regulatory compliance.

Respond promptly to urgent medication-related inquiries and issues during on-call hours. Document interventions and communications accurately in the patient records system.

**Qualifications**

Licensed Pharmacist in the state of Colorado. Experience in Long Term Care pharmacy practice preferred. Ability to work independently and remotely with minimal supervision. Strong communication skills for effective collaboration with healthcare teams. Familiarity with pharmacy software and electronic medical records. Commitment to maintaining patient confidentiality and professional ethics.

Work Environment This is primarily a remote position with rare requirements to come into the office.
