The Opportunity: We are currently seeking a highly organized, detail-oriented Intake Coordinator to join our Calhoun office. This role serves as one of the first points of contact for patients, families, referral sources, and healthcare partners.

What You'll Do

As an Intake Coordinator, you will play a vital role in ensuring patients are seamlessly connected to the care they need.

Responsibilities include

Managing incoming patient referrals Coordinating communication with hospitals, physicians, facilities, and referral partners Maintaining accurate referral records and documentation Scheduling admissions and coordinating logistics Collaborating closely with clinical and administrative teams Providing exceptional customer service to patients, families, and healthcare partners Supporting efficient and timely admissions processes What We're Looking For: Experience in hospice, home health, healthcare intake, or Medicare-related services Administrative or office support experience Strong working knowledge of Microsoft Office applications Excellent communication and customer service skills Strong organizational abilities and attention to detail Ability to multitask and prioritize effectively in a fast-paced environment A positive attitude and team-first mindset Location Candidates should reside in or near the Calhoun, Georgia area.
