Behavioral Health Case Manager

CHC is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction. CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974. We are currently seeking a dedicated Behavioral Health Case Manager to join our interdisciplinary treatment team.

The successful candidate will manage a caseload of incarcerated or detained individuals experiencing mental health, emotional, or substance use disorders. This role involves comprehensive assessment, treatment planning, intervention, and coordination of discharge and reentry services to support successful recovery and reintegration.

Key Responsibilities

Conduct psychosocial and risk assessments. Develop, monitor, and update individualized treatment and case plans. Act as a liaison between inmates/residents and internal medical/security staff or external community resources. Provide crisis intervention and de-escalation support. Coordinate pre- and post-release discharge planning (housing, employment, medication, and outpatient behavioral health).

Maintain accurate electronic health records and daily logs Qualifications & Skills Experience working with individuals with mental health, emotional, or substance use disorders, preferably within correctional or detention settings. Strong assessment and case management skills. Ability to develop and implement individualized treatment plans.

Excellent communication and interpersonal skills to effectively liaise with multidisciplinary teams and community resources. Proficiency in crisis intervention and de-escalation techniques. Familiarity with discharge and reentry planning processes. Competency in maintaining detailed and accurate electronic health records.
