Responsibilities:

Provide direct patient care to our residents, including administering medications, wound care, and vital sign monitoring

Collaborate with the healthcare team to develop and implement individualized care plans

Perform routine tasks such as vital sign checks, ambulating patients, and reporting any changes in patient condition

Document patient progress and communicate with family members or caregivers regarding patient status

Participate in quality improvement initiatives to ensure the highest level of care for our patients

Requirements:

Current LVN license in the state of Texas

High school diploma or equivalent required; Associates degree in Nursing or related field preferred

Ability to work a rotating shift, including evenings, nights, and weekends

Strong communication and interpersonal skills

Passion for delivering exceptional patient care
