Registered Nurse (RN)-Telephone Triage, PT, Every Other Weekend, Variable

Job Summary

Provides patient care, education, and guidance via phone by assessing symptoms, determining the appropriate level of care, and offering evidence-based advice through symptom-based triage guidelines. Ensures timely, safe, and effective care for patients of all ages while supporting care coordination such as appointment scheduling.

Collaborates with other clinicians, departments, or other members of the multi-disciplinary care team during patient encounters. Assesses patient’s symptoms, needs, or questions through a structured telephone assessment.

Utilizes clinical guidelines to determine urgency of concern and navigate patients to appropriate care pathways. Provides education, home-care instructions, or follow-up recommendations per departmental processes and guidelines. Schedules appointments for patients Maintains thorough, accurate documentation of patient assessment and recommendations.

Collaborates with clinicians or other members of the multi-disciplinary care team Performs other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education

• Associate degree in Nursing. Bachelor's degree in Nursing preferred Experience
• Two (2) years in Acute Care Nursing. Experience with Schmitt-Thompson guidelines or Telephone Triage experience preferred In Lieu Of NA Required Certifications, Registrations, Licenses Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working. Certified in nursing specialty
