Social Worker - Hospital Team

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Hospital team, based in Hull.

What will your responsibilities be?

• Assess patient needs to support emotional, social, and practical well-being during hospitalization.
• Coordinate discharge planning by arranging follow-up care, rehabilitation, or community services.
• Provide crisis intervention and emotional support to patients and families during medical emergencies.
• Advocate for patient rights and help navigate healthcare systems, benefits, and legal protections.

Qualifications and Experience:

To be successful in this role you must have.

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
