• Rockford, IL, Position Type: Full Time, Salary Range: $38.00 - $38.50Hourly, Job Shift: 1st shift, Responsibilities: Under the supervision of the Director of Nursing, our ADON is responsible for training, scheduling, and guiding the clinical team toward delivering exceptional care experiences. Conduct initial assessment of prospective residents Perform wellness assessments and develop individualized care plans based on the needs and preferences of each resident Deliver health promotion services, offer disease prevention counseling, and provide self-care education to residents Engage with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience Foster communication by regularly engaging with physicians, pertinent healthcare providers, and family members, ensuring the exchange of information on resident health status Contribute toward quality assurance and performance improvement initiatives Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences Comply with all rules and regulations regarding confidentiality and privacy of team members and resident information Perform any other duties as assigned or necessary Qualifications: Qualifications: Holds valid state licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) Minimum of one (1) year supervisory experience Exceptional written and verbal communication skills Prior senior care experience strongly preferred Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity Benefits: Advancement and professional growth opportunities Comprehensive medical, dental, and vision benefit packages EAP & wellness programs 401(k) with biweekly employer match* Outside-In Employee Referral Program Intentional work-life balance supported with competitive Paid Time Off* Self-service payroll & accuracy tools (Paycom) *Benefits and eligibility may vary based on employment status (full-time/part-time)
