Job description
To ensure and maintain the highest quality of care for our clients through our Care Professional workforce while building and maintaining excellent relationships with all external clients, customers, and field staff. Responsible for optimal clinical and financial outcomes, as well as stakeholder satisfaction, for an assigned caseload of clients.
This role is primarily field based with most work time spent in client homes and the community completing assessments, supervisory visits, and client/care professional support.
