Core Responsibilities

As a PRN RN in San Diego, you provide skilled nursing care in the home setting, supporting safe recovery and ongoing health management. You will complete focused assessments, develop and update individualized care plans, administer medications and treatments per orders, deliver patient/caregiver education, and coordinate with physicians and the interdisciplinary team. Accurate, timely clinical documentation and adherence to infection control and safety standards are essential.

Required qualifications/skills:

• Active RN license (California) and current BLS
• Home health or comparable clinical experience preferred
• Strong assessment, communication, and time-management skills
• Reliable transportation and ability to travel to patient homes

Your PRN Rhythm

Your day begins by reviewing the schedule and patient notes, then traveling to homes across the San Diego area. Between visits, you connect with the care team to confirm updates, clarify orders, and ensure each patient’s plan stays aligned with current needs. You’ll balance hands-on clinical time with concise charting, respond thoughtfully to patient questions, and adjust the day as priorities shift. The work is independent but well-supported, with clear communication and a steady focus on delivering consistent, safe care in the home.
