Mental Health & Addictions Service Coordinator, Here 24/7

This position will provide a range of services at the “front door” to our addictions and mental health system including intake, referral, crisis functions, as well as brief support to individuals who are waiting for ongoing service. The Service Coordinator supports Ontario Health West's Provincial Coordinated Access model by completing standardized screening and triage, managing referrals, and matching individuals to the most appropriate mental health and addictions services across the system.
