Job description
Licensed Practical Nurse (LPN) – PACE Flexible Scheduling Options: 4x10s, or 3x12s with rotating weekends Join Hope. West and be part of a team that is changing the way healthcare supports aging adults in our community. At Hope. West, we believe exceptional care begins with exceptional people. We are looking for a compassionate, skilled Licensed Practical Nurse (LPN) to join our growing PACE (Program of All-Inclusive Care for the Elderly) team.
PACE provides comprehensive, coordinated healthcare and supportive services that allow older adults with complex medical needs to remain living independently in their homes and communities for as long as possible. As an LPN, you will play an important role in delivering hands-on care, supporting participants and families, and collaborating with an interdisciplinary team committed to providing person-centered care.
What You'll Do
As a PACE LPN, you will provide direct nursing care and clinical support to participants in their homes.
Responsibilities
Provide direct nursing care to PACE participants in accordance with individualized care plans and provider orders. Maintain accurate and timely clinical documentation. Follow all applicable regulations, policies, procedures, and standards of care. Why PACE? PACE is more than healthcare—it is a different way of caring for older adults.
Instead of focusing on one appointment or one health concern at a time, our interdisciplinary team works together to support the whole person. This collaborative approach allows you to build meaningful relationships with participants while working alongside professionals who share a common goal: helping people live with dignity, independence, and quality of life.
