Service Coordinator - Lancaster/Berks Counties

Summary We are seeking a Service Coordinator to join our team. This pivotal role involves coordinating services for individuals with disabilities and our older adult population, ensuring they receive the support necessary to enhance their independence and quality of life. The Service Coordinators responsibilities are: Development of the person-centered service plan that address a Participants identified areas of need, goals, intervention, and progress of goals.

Coordinate services for Participants in relation to their identified needs. Monitor authorized services through face-to-face in-home visits, phone calls, person centered planning team meetings, and contact with service providers.
