Position Summary

This full-time salaried leadership role is responsible for providing clinical, administrative, and operational supervision to the Care Management Team while promoting whole-person, integrated care.

Qualifications

Candidates must be: Provisionally or fully licensed as an LCSW, LCMHC, LPA, LMFT, or RN. Have at least 3 years of experience providing care management, case management, or care coordination. Experience working with individuals with co-occurring Behavioral Health and I/DD needs is preferred.

Key Responsibilities

Provide clinical and administrative supervision to a care management team consisting of up to 8 Care Managers and 3 Extenders. Lead staff recruitment, onboarding, training, performance management, discipline, and termination processes. Ensure quality service delivery and compliance with service definitions through activities such as: Facilitating case conferences.

Conducting record audits. Observing team performance. Participating in home visits and Child and Family Team (CFT) meetings. Ensure timely and accurate documentation of all Care Management activities in accordance with policy and program requirements.
