Complex Care Nurse

We are seeking an experienced and compassionate Complex Care Nurse to join our dedicated community healthcare team supporting children and adults with complex health needs in their own homes and community settings You will play a vital role in overseeing clinical care plans , supporting healthcare assistants , and ensuring high-quality, person-centred care is delivered to every client.

Responsibilities

Conduct comprehensive clinical assessments for clients with complex needs (e.g., tracheostomy, ventilation, PEG feeding, seizure management). Develop, review, and implement individualised care plans in line with best practice and CQC standards. Deliver hands-on clinical support to clients and provide clinical training to support staff in the community.

Liaise effectively with multidisciplinary teams , including GPs, community nurses, therapists, and families. Monitor and audit care quality to ensure compliance with clinical governance and safeguarding policies .
