Designated Coordinator - Sub-Contracted Foster Care
Job description
The Designated Coordinator for Sub-Contracted Foster Care Providers is responsible to ensure that person-centered services are provided to service recipients that live in Sub-Contracted Foster Care Provider 203-AFC Licensed Homes. Duties include ensuring that services are provided in a manner consistent with MN 245D and the Positive Support Rule.
