• Patchogue, New York Job Summary: We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Swan Lake Nursing & Rehabilitation in Patchogue, New York. As an LPN, you will play a vital role in delivering high-quality patient care, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
