Seeking a Compassionate Primary Care Nurse Practitioner in Pell City, AL Summary: Our client is seeking a experienced and compassionate Nurse Practitioner or Physician Assistant to join their team in delivering high-quality care to our diverse and underserved patient population. The ideal candidate will diagnose, educate, and initiate treatment in an outpatient clinic setting, collaborating with a multi-disciplinary team to promote health and prevent disease.

Core Responsibilities: Comprehensive Clinical Care: Provide comprehensive primary care services to patients of all backgrounds, including preventive, wellness, and acute care visits. Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines.
Team-Based Care & Coordination: Collaborate with nurses, care coordinators, behavioral health providers, and specialists to manage care transitions, ensuring continuity, particularly for medically complex patients. Patient Advocacy & Communication: Build trusting, long-term relationships through effective, empathetic communication.
Educate patients and families on disease management and healthy lifestyle changes. Efficient Workflow & Technology Adoption: Utilize Electronic Health Record (EHR) systems to maintain accurate, up-to-date, and detailed documentation in the electronic medical record system.
