Community Health Worker

Description Join Our Mission Park Du. Valle Community Health Center is seeking a dedicated and compassionate Community Healthcare Worker to join our team. The ideal candidate will play a vital role in promoting health and wellness within the community by providing support, education, and resources to individuals and families.

This position requires strong interpersonal skills and a commitment to improving the health outcomes of diverse populations.

Key Responsibilities

Provide health education and resources to community members. Assist individuals in accessing healthcare services and navigating the healthcare system. Conduct outreach activities to promote health initiatives and programs. Collaborate with healthcare professionals and community organizations to address health disparities.

Monitor and report on community health trends and needs. Support patients in managing chronic conditions and adhering to treatment plans. Facilitate workshops and support groups to enhance community health awareness.

Requirements

High school diploma or equivalent; relevant certification or degree in healthcare or social services preferred. Strong communication and interpersonal skills. Ability to work effectively with diverse populations. Knowledge of community resources and healthcare services. Experience in health education or community outreach is a plus.

Strong organizational and time management skills. Compassionate and empathetic approach to patient care.
