Statement of General Purpose The Performance Management (PM) Data Coordinator is responsible for providing support to the CUA Cares Center in accordance with internal policies and procedures, the Philadelphia DHS Improving Outcomes for Children (IOC) Practice Guidelines, and PA DHS regulations. This position will complete electronic chart reviews, and will review, analyze, and distribute external audits and licensing inspection results.

The PM Data Coordinator will initiate and maintain specialized audits based on identified internal and external trends and findings. The PM Data Coordinator will also initiate and perform continuous quality improvement activities and trends reports, as needed. This position will collaborate closely with the Quality Improvement (QI) Director, QI team, and the CUA Cares staff.

The PM Data Coordinator will assist in creating internal and external plans of improvement. The PM Data Coordinator will also adhere to city, state, federal, and agency standards. The PM Data Coordinator also supports a percentage of the administrative needs of CPA. In addition to the above tasks, this individual will also be responsible for performing duties such as data collection and dissemination, risk management activities, service concern follow-up, quality check phone calls, incident and high-profile management, assist with compliance tracking, and special projects, as needed.

The PM Data Coordinator will also maintain the confidentiality of sensitive information. Examples of Essential Functions and Responsibilities Completes and distributes timely completion of internal case file reviews. Reviews and distributes external licensing results. Completes ongoing trends reports and quality improvement measures based on internal and external findings.

Conducts interviews, completes quality related calls, and may conduct onsite visits, as needed. Assists with timely preparation of licensing inspections and activities of Phila DHS, PA DHS, and additional stakeholders, as needed. Assists with data collection, compliance reports, and dissemination, as needed. Travel to offsite locations for training, inspections, service concern follow up, meetings, and other activities.

Completes client and stakeholder interviews. Communicates with various service providers, and key stakeholders, as needed. Timely review and dissemination of HCSIS Reports and internal client incident reports. Provides feedback to the CUA Cares department regarding incident management and reporting requirements. Creates and facilitates quality improvement related training at New Hire Orientation and ongoing.

Assists with data-related requests and quality improvement measures within Caring People Alliance, The CUA Cares Center, Phila DHS, PA DHS, and any additional funding sources, as needed. Investigates risk management concerns, staff-related service concerns, and reports the findings to the Quality Improvement Director.

Assists in identifying and developing plans of improvement and disputes for the agency based on audit findings. Solicits staff feedback and collaboration. Other duties as assigned. Ability to create and present data both verbally and written to various internal and external parties.

Ability to read, understand, and organize documents, such as: policies, procedures, regulations, audit tools, and reports. Knowledge of regulatory standards and the continuity of care within the social services environment.

Ability to review electronic records, internal and external audit findings, and trends reports. Ability to correspond professionally with internal and external stakeholders. Strong organizational, problem solving, and follow-up skills. Proficient with Microsoft Office Suite (PowerPoint, Word, and Excel) and ability to learn additional databases and data software applications.

Strong interpersonal and communication skills. Ability to establish and sustain strong working relationships. Minimum Requirements Bachelor’s degree within a related field. Two (2) years of experience in a social service setting. One (1) year of experience in data analysis and/or project management. Driver’s license with clean driving record and ability to drive own car.

Ability to work from home remotely within a quiet and confidential workspace. Must have home internet access ADA Component This role requires regular hearing, talking, walking, and sitting; occasional standing, driving, and lifting up to twenty-five pounds. Requires regular travel.
