What You’ll Do Follow each patient’s individualized care plan and physician orders.

Monitor patient progress and report changes or safety concerns. Teach patients and caregivers self-care techniques. Partner with the RN Case Manager to update care plans as patient needs change. Document medication regimens and maintain medication profiles according to agency guidelines. Communicate with physicians, patients, families, and the care team to coordinate care.

Help prevent avoidable hospitalizations and emergency room visits through proactive nursing care. What You’ll Need Current state license as a Licensed Vocational / Practical Nurse. Graduate of an accredited Certificate, Diploma, or Associate School of Nursing. At least 1 year of experience in a clinical care setting. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
