Case Manager Responsibilities: You will arrange safe post-hospitalization care, rehabilitation facilities, or home health services to prevent readmissions You will advocate for client rights, navigate insurance pre-authorizations, and respond immediately to crises such as housing instability or abuse You will evaluate patient or client needs, review medical/social records, and create tailored treatment or care plans Case Manager Qualifications: You must be qualified to independently complete an assessment within the scope of practice within your discipline (I.
