Job Description: Responsibilities: Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan. Performs patient rounding with an interdisciplinary team and participates in case conferences.

Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results. Evaluates patient's response and intervenes to ensure optimal patient outcomes QUALIFICATIONS: Palliative care experience desired - inpatient or outpatient Bachelor's degree, Nursing- Required Master's degree, Nursing- Required Basic Life Support-American Heart Association
