HOME I HOSPITAL TEACHER Kentucky Teaching Certificate Building Principal / Director of Pupil Personnel Not Applicable I. Provide instruction on a timely basis to students at home, or in hospital, as required by 704 KAR 7:120 2. Maintain data on home/hospital students and submit monthly and yearly reports. PERFORMANCE RESPONSIBILITIES: Provide home/hospital instruction on the basis of at least two (2) visits per week, one (I) hour instruction per visit to those students who cannot attend regular public school classes due to a physical, mental, emotional or social condition which prevents or renders inadvisable attendance at school. One and one-half (I �) hours of instruction per visit may be authorized for long term homebound students. Obtain student referrals for home/hospital instruction from the Principal or the Director of Pupil Personnel. Facilitate referrals from other sources (i.e. teachers, physicians) to the Principal, Director of Pupil Personnel. Facilitate the necessary completion of forms for admittance into the program. Maintain accurate attendance records as well as other student information. Complete a weekly visitation schedule and place it on file in the office. Obtain all necessary books and teaching materials for the appropriate instruction of identified pupils. Attend al committee meetings involving the admission and/or release of exceptional children placed in the home/hospital instruction program. Help to develop and implement the individualized education program (IEP) for exceptional children placed in the home/hospital instruction program. Report educationally relevant information maintained in pupil record to the regular classroom and/or special education teacher when the pupil returns to school. Maintain accurate mileage records incurred while providing home/hospital instruction in keeping with Board policies. Mileage for homebound instruction in extra mileage incurred as a result of the home visit. The hourly rate based on extra service schedule. Other duties as assigned by the Superintendent. TERMS OF EMPLOYMENT: Salary and work day to be established by the Board. EVALUATION: Performance of this job will be evaluated annually in accordance with provisions of the Board's policy on Evaluation of Professional Personnel.
