Description STATUS: Non-exempt (Full-Time 37.5 hours) Hourly Rate : up to $25.00 SUMMARY In this innovative program Care Managers bridge the gap between Adult Protective Services (APS) and community-based aging services. Elder Justice Care Managers work with APS to identify and serve older adults who would benefit from programs, entitlements, and opportunities that are available within the community.

Care Managers provide a holistic assessment of the clients’ needs by identifying health, financial, emotional, functional and environmental concerns, and they offer clients care options, solutions and guidance including connection with appropriate programs and systems. The Project Coordinator (PC) is a specialized Care Manager who works closely with APS to integrate aging services.

The PC acts as a consultant to APS staff at the Monroe County Department of Human Services concerning services, including those offered by Lifespan, for APS-referred adults age 60+. The PC is co-located at both the APS and Lifespan offices and is available for joint assessments with APS staff as appropriate. The PC acts as the lead Care Manager for the Elder Justice team by facilitating new referrals to the program and assisting with coordination of the overall care management work.

In addition, the PC carries a smaller caseload of clients and works with them to develop a care plan and connect with services.

DUTIES AND RESPONSIBILITIES

1.0 Provide care management to clients. Accepts referrals as assigned. Completes comprehensive in-home assessments to determine needs. Creates and implements client-centered care plans in conjunction with APS, as appropriate, that meet client needs and preferences. Provides information and referral, advocacy, benefit and entitlement counseling.

Records case activity in the Lifespan database designated for the program. 2.0 Arranges mental health interventions for older adults and their caregivers, when necessary. Conducts mental health screenings when appropriate. Provides supportive counseling to older adults. Works in tandem with APS to identify and provide linkage to appropriate ongoing mental health community services at intake.

3.0 Participates as a member of the inter-agency team. Acts in the capacity of lead Care Manager for program activities and assists in coordination of the activities of other care management staff assigned to the program. Participates in all Community Care Connections team meetings and trainings. Participates in all Lifespan staff meetings and trainings.

Participates in quality assurance activities. Provides input into the continued development of the Elder Justice program. Accepts and seeks guidance and clinical supervision. 4.0 Maintains up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies through reading, workshops and conferences.

Participates in training required for program activities including Motivational Interviewing, Trauma Informed Care and Racial Equity training. 5.0 Is aware of and actively supports the Agency’s initiative of being collaborative and community focused, innovative and culturally competent. 6.0 Other duties as assigned by supervisor.

Requirements

Bachelor or Master of Social Work or equivalent degree. EXPERIENCE: Two years in social work, with at least one year’s experience in geriatrics. Bilingual preferred but not required. PHYSICAL AND MENTAL REQUIREMENTS

  • Must be able to meet with clients in their homes.
  • A portion of the work is performed at a desk or at a personal computer station.
  • Verbal, written and telephone communication skills are required.
  • Strong written and computer skills needed.
  • Interface with all levels of personnel.
  • Ability to work in an Adult Protective Service environment and with older adult clients at risk of abuse, neglect, exploitation and self-neglect.
  • Ability to maintain composure under strict deadlines and difficult situations.
  • Ability to work in multi-cultural situations.
