Community Health Worker

POSITION SNAPSHOT Location: Clarion Community Health Center Schedule: Monday – Friday 8:30am – 5:00pm Employment Status: Full-Time WHY THIS ROLE Looking for a rewarding role where you can help individuals overcome barriers to healthcare and connect with important community resources? Primary Health Network is seeking a Community Health Worker to serve as a trusted link between patients, healthcare providers, and community services.

This position helps individuals access care, navigate available resources, and build the knowledge and support needed to improve their health and well-being.

WHAT YOU'LL DO

Help patients identify and overcome barriers to receiving healthcare. Connect patients with healthcare, social services, and community resources. Provide health education, coaching, social support, and advocacy. Assist with care coordination, appointment scheduling, transportation needs, insurance concerns, and system navigation.

Conduct patient outreach and follow-up, including telephone calls and home visits as appropriate. Reinforce education provided by healthcare professionals and help patients better understand healthcare information and resources. Build relationships with community organizations and participate in community outreach activities.

Maintain accurate and timely documentation in the electronic health record.

WHAT WE'RE LOOKING FOR

Bachelor’s degree in Social Work, Human Services, Healthcare, or a related field preferred, or at least 3 years of directly related work experience. Community Health Worker certification or willingness to become certified within the first year of employment. Valid driver’s license. Strong communication, organization, critical-thinking, and problem-solving skills.

Ability to work independently and build professional relationships with patients, providers, staff, and community partners. Experience with electronic medical records (EMR) and proficiency in Microsoft Outlook, Word, and Excel.

COMPENSATION

$18 per hour, however, PHN recognizes experience, education, credentials, and site location when determining wage, as applicable.
