What you’ll do Provide skilled nursing care, comprehensive assessments, and ongoing treatment where residents live. Develop, document, and update individualized plans of care, including admission and recertification assessments.

Teach patients and families about medications, self-care, nutrition, and managing health conditions. Coordinate with physicians, community staff, families, and our care team when a patient’s needs change. Provide clinical guidance and support to LPNs/LVNs and Home Health Aides. Participate in community wellness events, health fairs, and resident education sessions.

Maintain timely clinical documentation and follow infection prevention practices.
