We are currently seeking a Licensed Medical Social Worker (LMSW)to join our team for per diem home health visits. Key Responsibilities

for the Licensed Medical Social Worker LMSW:**

Conduct social work assessments and develop personalized care plans for home health patients.

Provide counseling and support to patients and families to help them adjust to illness, injury, or the challenges of aging.

Assist patients in connecting with community resources, services, and support networks.

Collaborate closely with the interdisciplinary healthcare team, including nurses, therapists, and physicians, to ensure holistic patient care.

Offer crisis intervention, advocacy, and guidance to patients and their families as needed.

Maintain thorough and timely documentation of services provided in compliance with healthcare regulations.

Minimum Qualifications

for the Licensed Medical Social Worker LMSW:**

Required:** Current MSW license in the state of Pennsylvania.

Prior experience in home health, hospice, or a similar healthcare setting preferred.

Knowledge of local community resources and support services in Lackawanna, Wyoming, Wayne, and Susquehanna counties.

Strong communication skills and the ability to work both independently and within a team.

Reliable transportation and a valid driver’s license.

CPR and First Aid certification preferred but not required.
