You'll care for patients ages 2 and older , providing preventive and acute care, managing chronic conditions, performing occupational health assessments, and supporting workplace wellness and safety. This opportunity combines relationship-based primary care with a variety of occupational health services, allowing you to make a meaningful impact on both individual health and workforce well-being.

Position Hours: PRN Clinic Operating Hours: Monday

• Wednesday | 7:00 AM - 5:00 PM; Thursday | 7:00 AM - 4:00 PM; and Fridays | 7:00 AM - 12:00 PM What You'll Do Provide comprehensive primary care and diagnostic services through in-person and virtual visits, as clinically appropriate Assess, diagnose, and manage acute and chronic health conditions for patients ages 2+ Develop individualized treatment plans and prescribe appropriate medications and therapies Promote preventive care, wellness, and chronic disease management through patient education Collaborate with medical assistants and other clinicians to support coordinated, patient-centered care Maintain accurate clinical documentation and adhere to applicable medical, regulatory, and HIPAA requirements Contribute to a positive, collaborative clinic environment focused on quality care and patient outcomes What You'll Bring Active, unrestricted Nurse Practitioner license in New Hampshire Current Family Nurse Practitioner (FNP) board certification Active DEA registration Experience providing primary care to patients ages 2 and older 2+ years of primary care experience preferred Experience with occupational health services, including workplace physicals, screenings, and testing, preferred Knowledge of HIPAA requirements and commitment to patient confidentiality Ability to successfully complete required credentialing Commitment to preventive care, chronic disease management, and workplace wellness The CareATC Core4 Committed
