Job description
The RN Case Manager is responsible to plan, organize and direct the Lifespark services while insuring compliance with regulatory requirements and Lifespark best practices including but not limited to completing, starts of care, care plan development and ongoing support, client satisfaction, caregiver supervision and engagement and on-boarding. The professional nurse builds from the resources of the community & care team to plan and direct services to meet the needs of individuals and families within their homes and communities.
Position: RN Case Manager for Home Health Wage Range: $88,000-$90,000 annually, DOE Service Area: Multiple Cities in a Regional Assignment Schedule: Full-Time 1.0 FTE, Monday – Friday 8:00am-5:00pm (with rotation every 6th weekend and 2 holidays/year)
