JOB DESCRIPTION WOUND CARE NURSE (LPN) Shift: 8:00 AM- 4:30 PM Department: Nursing JOB RELATIONSHIP: Supervised By: Director of Nursing, Assistant Director of Nursing, and/or Clinical Care Coordinator Workers Supervised: CNA’s, and LPN’s per wound care responsibilities. WORK SCHEDULES: All positions at Baptist Village, Inc.

QUALIFICATIONS

Graduation from an accredited school of nursing Current LPN licensure by State Board of Nursing Experience with administrative techniques and leadership capacity Obtain wound care treatment certificate of completion (on the job) Available to work holidays and weekends as work demands Good physical health condition and pass a physical examination ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Work with physicians and other medical professionals in obtaining orders, plan of care, and providing care updates to personnel as required Follow wound care and skin assessment protocols using the Braden Scale Conduct weekly skin assessments according to the protocol Discuss skin issues and changes in condition (antibiotics) with hall nurses daily, to include reviewing documentation and orders for dressings/treatments Assess and change any wound, pressure ulcer, incision, or any other skin integrity treatment and complete documentation per appropriate wound/skincare protocol Obtain cultures to assess wounds and injuries as needed Evaluate wounds and injuries for infections or other illness factors that cause wounds Coordinate care and recommendations to rehab and nutritional staff Team leader for HANDS program; practice and teach HANDS per protocol Participate in care plan/resident care team meetings as requested Provide skin reports to leadership as requested Work with leadership to determine the best course of action for wound care/supply financial coverage (i.

e., Medicare/Part B) Educate residents and family members of wound status and teach causative factors and preventive measures using terminology easily understood Educate and update families weekly for facility-acquired pressure ulcers and routinely for others Educate direct care staff about wounds, properly caring for the wound and current interventions/preventive measures Attend educational opportunities to stay up-to-date on current wound care techniques and best practices for identifying wound types, skin conditions, the staging of wounds, and appropriate treatment plans May be directed to participate in routine LPN responsibilities as the need arises The associate is authorized to have unlimited access to resident health information as needed to provide treatment, for billing purposes, or for the purpose of healthcare operations.
