Physical Therapist (Full-Time or PRN)

• Appleton, WI, Salary Range: Undisclosed, We are inviting you to apply for a Physical Therapist position. We have full-time and PRN opportunities available. The Physical Therapist will establish and carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, communication and discharge planning to maximize a patient's progress toward achieving functional goals. Duties/Responsibilities: Designs and implements a plan of care for patients based on a thorough assessment of needs and incorporates participation by significant others as appropriate. Provides direct patient care, evaluates patient functional needs and outcomes, adjusts care plans, consults with other specialists, and actively participates in the discharge planning process. Possesses a working knowledge of patient’s insurance coverage, verification and completes required insurance documentation in a timely manner. Conducts therapy practice within defined standards of care for physical therapy. Directs and supervises physical therapy assistants as allowed by professional license. Directs the assistance of other support staff as allowed to achieve patients’ functional goals. Communicates effectively with other professional and support staff to achieve positive outcomes and interdisciplinary team discharge planning. Practices facility and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards, including cleaning of therapy equipment, workstation and shared work areas. Completes and documents interventions in a timely manner. To include: Evaluations Goals Plan of Care Treatment Encounter Notes Progress Notes Recertifications Discharges Billing Maintains Certifications and Physician signatures. Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature, continuing education units, State licensure, and certifications as applicable. Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community. Additional duties as assigned Qualifications: Required Skills/Abilities: Knowledge and skills to care for the physical and functional needs of patients. Ability to remain calm in stressful situations. Professionalism, accuracy, and dependability. Excellent customer service and patient care skills.

Education and Experience

Must possess a Degree in Physical Therapy from an accredited college or university. Must possess a current, unencumbered license to practice as a Physical Therapist in the State of Wisconsin.
