For a placement request to be considered, we require a few things: The Student Placement Request form to be filled out and returned along with: Proof of Identity (Original to be sighted, not a copy) Next of Kin Details NDIS Worker Screening Check number The student Placement request form will be sent to you after your application*** Evidence of attending the Educational Institution (EI) A letter confirming Student is: Currently Studying covered by EI's Insurance a copy of the Learning Plan/Workbook Applicant Is not listed on the Aged Care or NDIS banning registers.
