About the role

We are seeking a dedicated Long-Term Care (LTC) Pharmacist to join our fast-growing LTC pharmacy team. As an LTC Pharmacist, you will play a critical role in ensuring safe, accurate, and timely medication management for residents in LTC Facilties What you'll do Medication Verification & Clinical Review Perform final verification of prescriptions, ensuring accuracy in drug, dose, directions, and day supply.

Conduct DURs (Drug Utilization Reviews), allergy checks, interactions, and therapeutic duplications. Review MARs, facility orders, and provider instructions for clarity and compliance. Clinical & Patient-Centered Care Provide clinical recommendations to providers and facilities. Review PRN and routine medications for appropriateness.

Support high-risk medication monitoring (anticoagulants, insulin, psychiatric meds, etc.) Participate in care-plan reviews or medication audits as needed. Assist in Consulting Pharmacist duties related to Medication Regimen Reviews Regulatory & Compliance Ensure compliance with LTC regulations (Medicare/Medicaid, CMS, DEA, State Board of Pharmacy).

Maintain accurate records for CII-CV medications, perpetuals, and emergency kits. Oversee return-to-stock, expired medications, and documentation required for audits. IMMUNIZATION CLINIC: Assisting with setup and break-down of wellness/immunization clinics. Transporting vaccine(s) and sharps Assisting with completion of immunization consent forms Proficient in intramuscular injection technique and subcutaneous injection technique Administer vaccines (per manufacturer instructions) and educates clients and caregivers about age-specific immunizations, precautions/contraindications, and possible adverse reactions Transport of vaccine(s) and maintaining the cold chain Proper biohazardous waste transport and disposal Maintains appropriate infection control standards and precautions Maintains compliance with wellness specific policies and procedures Emergency management, i.

e., adverse reactions, anaphylaxis. Reporting of incidents and/or injuries/needle sticks Proper biohazardous waste process/protocols Proper utilization of PPE Qualifications Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BSPharm). Licensed in State of Delaware Strong knowledge of LTC pharmacy regulations (Medicare/Medicaid billing) Excellent communication skills with nurses, providers, and internal teams.
