Adult Community Case Manager

Woodfords Family Services is seeking a compassionate and qualified Adult Case Manager to join our Community Case Management team. This is a full-time (40 hours per week), salaried position offering a comprehensive and competitive benefits package. This role is based out of our Westbrook, ME office and includes required travel throughout Cumberland County to support individuals and families in their communities.

Position Summary

Adult Case Managers at Woodfords play a vital role in supporting adults with intellectual and/or developmental disabilities by promoting independence, stability, and meaningful community engagement. Case Managers work collaboratively with individuals, families, and service providers to identify needs, coordinate supports, and connect consumers with appropriate resources.

Through a person-centered planning (PCP) approach, the Case Manager serves as a key advocate and communicator—ensuring services are aligned with individual goals and that all areas of development are supported with dignity and respect. This role emphasizes partnership, accountability, and empowerment to help individuals build sustainable, fulfilling lives.

Opportunity Details Full-time, 40 hours per week Monday through Friday, 8:00am-4:00pm $44,000(+) salary - commensurate with education and experience Monthly incentive opportunities starting at $350 , totaling a minimum of $4,200+ annually , with ability earn even more - in addition to base salary Qualifications Bachelors Degree from an accredited four-year institution of higher learning with a specialization in psychology, behavior health, social work, special education, counseling, rehabilitation, nursing, or a closely related field.

At least (1) year of experience in the field of social/human services. Valid State of Maine driver's license and have use of a personal vehicle and proof of auto insurance. Good organization, communication, decision-making, and human relation skills.
