Hospice RN Case Manager Southeast Metro

Job description
The Hospice RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Salary Range: $88,000-$94,600 annually depending on experience Service Area: Southeast metro. Plus, mileage reimbursement of 72.5 cents per mile. Schedule: 1.0 FTE, Monday – Friday 8:00am-5:00pm (with 5 weekend shifts per quarter, 2 holidays/year and occasional on call as needed to fill shifts)
