Hospice - On Call Social Work Clinician (weekends)

Our Hospice team has immediate openings for part time/on-call licensed clinical social workers to support the admissions and psycho-social needs of end-of-life patients and their families on weekends. Functions: Provide social work services to clients/family in their place of residence, to include evaluation, psychosocial assessment and reassessment of emotional factors impacting the client/family’s ability to cope, formulating and implementing a plan of care, and providing crisis intervention.

Attend Hospice admissions with Admissions Nurse, coordinating accompanying intake documentation. Assist families with identification of available community/agency resources and make referrals as appropriate. Enter all relevant information into the EHR. Position Qualifications: An active State of Maryland LMSW is required.

An LCSW-C is preferred. A minimum of one year of experience in a hospice program or related experience with geriatric population A valid Driver’s License Reliable transportation to make visits within Montgomery County Must be available to work every other Saturday and Sunday from 8:30am-5pm.
