Social Worker (MSW)

Position Summary

Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho‑social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers.

This position is Monday-Friday 7:30am-4:00pm with no nights or weekends! Location: Lower Burrell, PA Required Education: Master’s Degree in Social Work (MSW) from an accredited school of social work. Required Experience: Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience preferred in a hospital, long term care or community setting.

Ability to work independently and be self-directed. Good problem-solving skills. Must have a compassionate and empathetic attitude for the frail older adults.
