Responsibilities

Conduct assessments and develop individualized care plans for residents. Provide counseling and support to residents and their families during difficult times. Facilitate group therapy sessions to promote emotional well-being. Collaborate with healthcare teams to ensure comprehensive care delivery. Manage case files and maintain accurate medical records as required.

Assist residents in accessing community resources and services. Implement crisis management strategies when necessary.

Requirements

Valid state licensure as a Social Worker. Experience in clinical counseling or case management preferred. Strong communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team.
