Hospice LPN Grace Hospice | Minneapolis, MN Pay Range: $28.00–$35.00 per hour, based on experience Nursing With Purpose Looking for an LPN role where you can build meaningful relationships and provide personalized care that truly matters? Grace Hospice is looking for a compassionate Licensed Practical Nurse to join our interdisciplinary care team.

As a Hospice LPN, you’ll provide hands-on nursing care focused on comfort, dignity and quality of life. Working closely with the RN Case Manager and other members of the hospice team, you’ll help manage symptoms, provide treatments, educate families and respond to changing patient needs wherever they call home. What You’ll Do Provide direct nursing care based on each patient’s individualized plan of care. Assess comfort, safety and changing needs and communicate updates to the RN Case Manager.

Support the management of acute and chronic end-of-life symptoms and medications. Provide wound care, personal care and assistance with feeding as needed. Educate and support patients, families, caregivers and facility partners. Collaborate with the interdisciplinary team to provide coordinated, responsive care. Participate in bi-weekly interdisciplinary group (IDG) meetings.

Complete required documentation and follow patient safety, infection prevention, confidentiality and patient-rights practices. What We’re Looking For Current Minnesota LPN license. Graduate of an accredited practical nursing program. Previous nursing experience preferred; hospice or home health experience is a plus. Strong assessment, communication and critical-thinking skills.

Compassionate and respectful approach to patient and family care. Ability to work independently while collaborating with the interdisciplinary team. Comfortable using electronic health record systems. Valid Minnesota driver’s license, auto insurance and reliable transportation.
