What You'll Do

Complete speech-language pathology evaluations in patients' homes Develop individualized plans of care Treat speech, language, communication, cognitive, and swallowing disorders Provide dysphagia treatment and swallowing education as appropriate Help patients improve functional communication and cognitive skills Educate patients, families, and caregivers Monitor progress and adjust treatment plans as needed Complete reassessments and discharge planning Coordinate care with the interdisciplinary team Complete accurate and timely documentation using WellSky What We're Looking For Active Virginia Speech-Language Pathologist License Required Graduate Degree in Speech-Language Pathology from an accredited program Current ASHA Certification (CCC-SLP) Preferred Valid Driver's License and reliable transportation Strong clinical and communication skills Ability to work independently in patients' homes Home Health Experience Preferred, but Not Required WellSky Experience is a Plus Why Human Touch?
