Case Manager-Social Worker Trainee The Central Mississippi Planning & Development District has been promoting regional excellence since 1968. JOB DESCRIPTION Classification: Non-Exempt Department: Medicaid Waiver Reports to: Case Management Director, HR Manager Summary/Objective The Case Manager-Social Worker Trainee position is designed to be a temporary position for the purpose of licensing exam preparation and observation of the Case Manager-Social Worker position which is involved in the coordination of the Case Management Program and consists of planning, arranging for, and coordinating of appropriate home and community-based services which shall be provided to clients in a timely and cost-effective manner.

Incumbent assigned to this position maintains considerable contact with service providers, consumers of services, civic groups, and the public. This position is designed to transition into a non-trainee position after 12 weeks upon passing the ASWB licensing exam.

Essential Functions

Follow prescribed study and exam prep program steps, performing all recommended activities, practice tests, and appropriate study structure to ensure readiness for final Association of Social Work Boards (ASWB) Licensed Social Worker (LSW/BSW) licensing exam which is to be taken within 12 weeks from date of hire.

Adhere to all requirements for Social Work Licensure in the state of MS and complete said requirements with a passing score within 12 weeks from date of hire. Continued employment with CMPDD is contingent on passing the ASWB exam. Observe teams consisting of Registered Nurses and Social Worker case managers in the team approach to screen referred clients and determine eligibility.

Accompany various assigned teams during on-site visits, observing them while they use prescribed skills to assess the client, formulate nursing diagnoses and needs of the client, note medical diagnoses, establish plan of care, implement plan of care after eligibility has been established and approval received from appropriate sources, and evaluate results of the plan by monitoring clients at least monthly.

Work Environment While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is regularly required to stand, walk, sit; use hands to finger, handle or feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 20 pounds. Specific vision abilities required by this job include close vision distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m. Travel Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected. Required Education and Experience Bachelor’s degree in social work or related field and two (2) years of full-time experience working in direct service to aged and disabled individuals. Candidates with less than two (2) years of experience will require at least 90 days of orientation under the supervision of an established case manager that has two (2) years of waiver experience. Additional Eligibility Qualifications Thorough knowledge of the principles, procedures, and techniques of professional social work; knowledge of community resources available to individuals and families; knowledge and skill in interviewing and assessment techniques; ability to work in a team relationship; and knowledge and understanding of psychological, social, health and economic factors influencing the attitudes and behavior of individuals and families, especially as they relate to the elderly. Ability to establish and maintain effective working relationships with others. Excellent verbal and written communication skills. Basic knowledge of word processing programs, spreadsheets, email platforms and other communication tools. Must have and maintain a valid Mississippi driver's license.
