Position Summary

Responsible for providing services to both inpatients and outpatients, including assessment, treatment, planning, implementation and related documentation. Position Requirements: Education: Graduate of accredited School of Physical Therapy. Licensure: Licensed as a Physical Therapist in the State of Illinois. Certifications: Certified in CPR through the American Heart Association.
