As an LPN Care Coordinator, you'll help patients successfully navigate the healthcare continuum by supporting care transitions, chronic disease management, population health initiatives, and post-discharge follow-up activities that improve outcomes and enhance the patient experience.

What You'll Do

Collaborate with providers, care teams, and departmental staff to identify, manage, and support high-risk patients across the continuum of care. Meet with hospitalized patients, as appropriate, to assess discharge needs, provide education, and facilitate a safe transition of care. Conduct readmission risk assessments and follow-up evaluations for patients discharged within the previous 30 days to identify opportunities for intervention and improved outcomes.

Assist providers in identifying patients at moderate to high risk for readmission and implementing strategies to reduce avoidable hospitalizations. Perform post-discharge outreach calls to assess patient status, reinforce discharge instructions, identify barriers to care, and coordinate necessary follow-up services. Maintain timely, accurate, and professional communication with patients, providers, care teams, and other internal and external stakeholders to support safe, high-quality patient care.

Manage daily worklists and assigned care coordination activities, ensuring appropriate follow-up, documentation, and communication. Support compliance with Rural Health Clinic (RHC), Joint Commission, and other applicable regulatory and accreditation standards. Participate in payer-sponsored quality initiatives, meetings, and educational programs as assigned.

Perform chart abstraction, quality data collection, reporting, and data submission activities to support quality improvement and performance initiatives. Document care coordination activities accurately and timely within the electronic health record and other applicable systems. Assist with quality improvement efforts focused on enhancing patient outcomes, closing care gaps, and improving population health measures.

What You'll Bring

Required Qualifications Current Licensed Practical Nurse (LPN) licensure. Strong communication and interpersonal skills. Ability to coordinate care across multiple settings and providers. Knowledge of chronic disease management and care coordination principles. Commitment to patient-centered care and quality outcomes.

Strong organizational and time-management abilities. Preferred Qualities Care coordination or case management experience. Population health experience. Ambulatory or primary care experience. Experience working with high-risk patient populations. Knowledge of value-based care initiatives and quality measures. Physical Requirements Frequent communication with patients, providers, and care teams. Computer-based documentation and reporting activities. Periodic patient visits and care coordination activities. Education, Certifications & Licensure Current Licensed Practical Nurse (LPN) licensure required. Experience in healthcare, care coordination, or population health preferred.

Current Basic Life Support (BLS) certification preferred. Continuing education and competency requirements maintained as required.
