Outreach Specialist Worcester MA

Requirements

• You have 1-2 years experience with patient navigation, management, or any kind of direct service provision
• You have a Massachusetts Unrestricted Driver�s License and Car
• Provide telephonic, written, digital and face to face (in individual's homes) outreach and engagement services for each eligible member.
• Priority given to Spanish Bilingual candidates.
