You'll care for patients ages 2 and older, managing acute and chronic conditions, developing individualized treatment plans, and empowering patients through education and ongoing support. Position Hours: 20 hours per week Clinic Operating Hours: Monday

• Friday | 8:00 AM - 5:00 PM Work Schedule: To be determined within clinic operating hours.

What You'll Do

Provide comprehensive primary care through in-person and virtual visits, as clinically appropriate Assess, diagnose, and manage acute and chronic health conditions for patients ages 2+ Develop individualized treatment plans and prescribe appropriate medications and therapies Promote preventive care, wellness, and chronic disease management through patient education Collaborate with physicians, medical assistants, and other clinicians to support coordinated, patient-centered care Maintain accurate clinical documentation and adhere to applicable medical, regulatory, and HIPAA requirements Contribute to a positive, collaborative clinic environment focused on quality care and patient outcomes What You'll Bring Graduation from an accredited Physician Assistant program Active, unrestricted Physician Assistant license in the state of practice Current NCCPA certification Active DEA registration Experience providing primary care to patients ages 2 and older 2+ years of primary care experience preferred Knowledge of HIPAA requirements and commitment to patient confidentiality Ability to successfully complete required credentialing Commitment to preventive care, chronic disease management, and patient education The CareATC Core4 Committed
