Case Manager
Job description
Essential Duties and Responsibilities
Client Assessment and Service Coordination Identify client needs, strengths, barriers, and available support systems. Assist clients in accessing appropriate mental health, medical, social, and community services. Coordinate referrals and follow up to determine whether services were initiated and client needs are being addressed.
Collaborate with clinical staff to support individualized treatment and service planning. Assist clients with navigating community resources, including housing, transportation, employment support, benefits, and other eligible services. Monitor changes in client needs and communicate significant concerns to the appropriate clinical personnel.
Client Support and Advocacy Establish professional, respectful relationships with clients and support person-centered care. Encourage client participation in treatment and recovery planning. Advocate for access to appropriate services while respecting client rights, preferences, and confidentiality. Coordinate communication with families, providers, and other authorized parties when appropriate.
Recognize potential crisis situations and promptly follow agency procedures for escalation and referral. Documentation and Medicaid Compliance Complete accurate, timely, and individualized service documentation in the agency's electronic health record. Document services provided, coordination activities, referrals, client responses, and progress toward identified goals as required.
Ensure documentation supports the service delivered and complies with applicable Medicaid, managed-care, and agency requirements. Follow applicable confidentiality, HIPAA, client rights, incident reporting, and record-retention policies. Participate in audits, quality assurance reviews, and corrective actions when required.
Meet established productivity and documentation deadlines without compromising service quality or compliance. Team Collaboration and Training Participate in clinical staffing, case consultations, team meetings, and required supervision. Maintain communication with the assigned supervisor and appropriate members of the treatment team.
Complete required onboarding, compliance, Medicaid service-delivery, and ongoing competency training. Maintain professional boundaries and perform duties within the scope of assigned responsibilities and qualifications.
Minimum Qualifications
Education and experience
consistent with the specific position, applicable Texas Medicaid requirements, and the agency's contracted program. Experience in mental health, behavioral health, social services, case management, or community-based support preferred. Strong written and verbal communication, organizational, documentation, and problem-solving skills.
Ability to work effectively with individuals from diverse backgrounds and with varying support needs. Ability to maintain confidential records and comply with agency policies and applicable laws. Valid driver's license and reliable transportation if required for community-based client services.
